SCOPE OF WORK: The Standing Rock Diabetes Program (SRDP) is developing a
fitness program, to educated and assist individuals in living a more active lifestyle, in
order to prevent diabetes and it's complications. This position is located in the SRDP
Fitness Building, Fort Yates, ND.
DUTIES AND RESPONSIBILITIES:
1. Will follow SRST Tribal Personnel and Financial Policies and Procedures. 2. Will work under the general supervision of the Standing Rock Diabetes
Program Director. 3. Will be responsible for documentation, client records, and submit timely
reports as required. 4. Will be responsible for keeping a safe exercise environment in the SRDP
Fitness Building. 5. Will be responsible for teaching and enforcing safety rules for equipment
usage and general exercise safety. 6. Will be responsible for reporting needed maintenance on allfitness equipment
and keeping fitness equipment and the facilities in clean working order. 7. Will assist in developing individual or group fitness plans for clients with
special needs. 8. Will be responsible to instruct low or high impact aerobics as requested. 9. Willdo blood glucose and blood pressure checks on individuals clients during
fitness outreach activities, or health screening events as requested. 10. Will coordinates activities with the Diabetes Team, other programs, and
schools.
QUALIFICATIONS:
1 . A 2-Year degree in Physical Education, Health Education, Nursíng, or similar
area is desired, experience will be considered. 2. Must have a valid drivers license, vehicle and liability insurance. 3. Must be willing to travel to all dístricts on the reservation to assist in fitness
activities. 4. Must be able to conduct self in a professional manner. 5. Must have the ability to communicate both orally and written to people and
agencies.

6. Must be willing to go through training and obtain certification as a physical
fitness specialist, CPR/First Aid, and others as required. (Example: Cooper
lnstitute's training) 7. Must be able to perform strenuous physical activities. 8. Must maintain confidentiality, breaking of confidentiality will be grounds for
dismissal.

No Tribal housing is available. The position is located at the Standing Rock
Diabetes Program in Fort Yates, ND. The incumbent will work under the superuision
of the Director of the Diabetes Program.
