Conducts outreach and referral for mental health and substance abuse services, provides case management to assist in resolving client barriers or access to appropriate services, assesses client need and makes referrals to external and internal resources. Provides client advocacy in administrative relief issues, filing for benefits, interpreting documentation or forms to ensure client rights in administrative meetings/hearings or appointments as needed, enrollment liaison to establish eligibility for service, provide notary service to organization, clients, and network/partner with agencies to meet client needs. Gathers data on client services, prepare and submits monthly and grant reports as required and transport clients when necessary.
