This position is located at the Pine Ridge Hospital, Pine Ridge, SD and serves as the Diabetes Educator Coordinator for the Pine Ridge Service Unit and community. The Pine Ridge Service Unit consist of the Pine Ridge Hospital, Kyle Health Center, Wanblee Health Center and three (3) field clinic locations. The purpose of the position is to provide planned educational experiences, with emphasis on diabetes, as an intergal part of patient care. Through patient education the patient and family will be assisted in acquiring the knowledge, attitudes, and skills necessary to accomplish diabetes self-management of their health and wellness. Educational planning will be intiated upon provider referral or entry into active DM patient registry. The minimum requirement for this position is licensure as a Nurse with a Bachelors Degree in Nursing who has current certification as a Diabetes Educator (CDE) or is actively pursuing eligibility requirements for CDE.
Identifies individual patient needs and assists the patient and the patient's family in establishing treatment goals by counseling and recommending appropriate treatment modalities to satisfy the objectives of therapy. Participates in the diabetes program with multidisciplinary team members to assess and provide culturally relevant care for patients living with diabetes. Provides technical assistance and guidance in planning, implementation and evaluation of diabetes continuing education programs for all nursing personnel at the Service Unit; coordinates professional education training programs on a continuous basis to IHS diabetes team members at the Hospital and Health Centers through participation in planning and implementing the diabetes M&M conferences. Participates in the analysis and interpretation of collected data by relating results of knowledge diabetes and health care delivery systems which might cause or explain any variations Routines duties shall include providing nursing care services to individuals eligible for services provided by the IHS in homes, schools, clinics, job sites, and other community locations within the Service Unit. Performs all other related duties as assigned.

QUALIFICATIONS REQUIRED:
Certified Diabetes Educator. Applicants must
possess current certification as recognized by National Certification
Board for Diabetes Educators or actively pursuing eligibility requirements for CDE.
Registration: Applicants must have active current registration as a
professional nurse in a State, District of Columbia, the Commonwealth
of Puerto Rico, or a territory of the United States.
GS-11: 1 year of experience equivalent to at least the next lower
grade level
Evaluation of Experience: Experience must have equipped the applicant
with the particular knowledge, skills, and abilities to perform
successfully the duties of the position. At GS-9 and above, many
positions require experience in a specialty area of nursing.
Example of Specialized Experience: Independently develops a statement of philosophy, goals and objectives for patient education. Work covers a wide range of educational services from assessment through counseling, teaching, provision of basic nursing care and treatment. Uses various collaborative and independent processed to determine the needs of the Service Unit and the community in relation to diabetes education plans. Develops, implements and evaluates the diabetes program.
Education: Degree or diploma from a professional nursing program
approved by the legally designated State accrediting agency.
