JOB SUMMARY
Processes and analyzes charts for completeness, accuracy and relevance of diagnoses and procedures. Assigns ICD-9 & 10 codes to all diagnoses and procedures. Assigns CPT codes to appropriate procedures using coding guidelines. Completes deficiency slips and routes incomplete charts to providers. Reviews and analyzes all Patient Care Component (PCC) encounter forms for completeness, diagnoses/procedures, signatures and other patient related information. Initiates the billing process. In accordance with federal law, priority will be given to qualified Native American applicants.
DUTIES AND RESPONSIBILITIES include the following. Other duties as assigned.
Abstracts information from the medical record/PCC form, superbill and chargetickets.
Retrieves information from Resource Patient Management System (RPMS) to verify patient information, e.g., medications, laboratory, radiology or other ancillary services.
Assigns and sequences ICD-9 & 10, CPT codes and applicable modifiers for medication, supplies and durable equipment.
Determines the final diagnoses and procedures documented by provider.
Clarifies vague or incomplete diagnostic and/or procedural information with physicians and other heath care providers when necessary.
Assigns the DRG sub classification formula to Medicare/Medicaid and rehabilitation patient records.
Requires extensive interaction with the PCC System using mnemonics for entering and editing data for outpatient visits such as diagnoses, procedures, treatments, exam measurements, laboratory tests, historical data and immunizations.
Utilizes automated record system, data entry, encoder software.
Uses coding conventions, techniques, coding rules and regulations.
Keeps abreast of all coding trends and changes.
Provides education and updates to medical staff, office staff and other health care providers on coding changes, rules, regulations and guidelines.
Conducts quality control and improvement reviews by tracking and identifying inadequate documentation, communication program software and hardware problems and maintaining productivity logs.
Researches and resolves inquiries from insurance companies; physicians and patients regarding charge and coding verification.
Researches non-payment of accounts payable and/or accounts receivable.
Completes computer generated interfaces to assure that all coding is accurately reported to the billing department.
Designs and runs reports for administrators; health care providers and outside agencies using the department’s computer system as needed.
Ensures confidentiality in accordance with the Privacy Act of 1974, Alcohol and Drug Abuse Patient Records, Freedom of Information Act, HIPAA and other pertinent Federal regulations.
Attends training and conferences to meet requirements for maintaining certification.
Requires limited travel to fulfill job responsibilities.
GENERAL REQUIREMENTS
Must have a valid driver’s license and be insurable.
Must be able to pass a drug and alcohol screening.
Must be able to pass a background investigation.
QUALIFICATIONS
Experience
High School diploma, equivalent GED or higher.
Minimum CPC or equivalent certification required.
One or more years of professional experience with proficiency in coding of medical records using ICD-9 and CPT coding for physicians preferred.
Demonstrated knowledge of medical terminology, anatomy, physiology required.
Basic knowledge of medical procedures and disease stages/processes required.
Necessary Knowledge, Skills and Abilities:
Knowledge of health information management theory, principles, techniques, concepts and policies to analyze medical records.
Knowledge of medicolegal aspects of health information management.
Skill in data collection.
Knowledge of legal regulations and requirements pertaining to medical confidentiality, specifically the Privacy Act of 1974 and HIPAA regulations.
Ability to perform qualitative and quantitative review, analysis and technical evaluation of medical records.
Skill in computer literacy.
Ability to maintain a high level of confidentiality.
Exceptional attention to detail.
Ability to speak and write clearly, concisely and effectively and ability to read, understand and listen to information and ideas written and oral.
Ability to effectively organize workload and manage time.
Ability to deal with people in a manner that shows cultural sensitivity, tact and professionalism.
