JOB SUMMARY
Works under the direct supervision of the CHR Manager and in conjunction with the Community Health Nurse to help improve the health status, health knowledge, attitudes and practices of tribal community by promoting, supporting and assisting with health assessments, home assessments, vital sign measurements, diabetes blood testing, medical treatments (with supervision), medical transportation, health education and prevention. Works with the elders and children. In accordance with federal law, priority will be given to qualified Native American applicants.
DUTIES AND RESPONSIBILITIES include the following. Other duties as assigned.
Assists with special projects and conducts health activities.
Assists clients with disabilities with medical conditions.
Visits clients in their homes.
Performs vital sign measurements.
Assists with patient Activities of Daily Living.
Functions as a home health assistant as needed.
Performs diabetic foot exams (visual and sensory).
Performs diabetic blood testing.
Performs minimal patient assessments.
Provides general health education.
Provides education in the use of such things as: walkers, crutches, wheelchairs and canes.
Provides minimal nutritional education.
Makes referrals for needed services.
Transports clients for medical services.
Explains available health programs and policies.
Fills in for Clinic Receptionist as needed.
Assists with community health service clinics.
Assists with community health promotion and disease prevention events.
Enters diagnostic patient data into the PCC program.
Maintains official patient medical records.
Records information that generates critical data to the Federal Government for funding or various programs
Assists clients in filling out eligibility forms for services.
Attends training and conferences to meet requirements for maintaining certification.
Requires frequent travel to fulfill job responsibilities.
GENERAL REQUIREMENTS
Must have a valid driver’s license and be insurable.
Must be able to pass a drug and alcohol screening.
Must be able to pass a background investigation.
QUALIFICATIONS
Experience
High School diploma, equivalent GED or higher.
Active CNA certification required.
CPR, First Aid and CHR certifications required but can obtain once employed.
Experience or knowledge in health care.

Knowledge of the Native American community is helpful.
Demonstrates respect for and awareness of Native American customs and traditions.
