JOB SUMMARY:
The Service Coordinator is responsible for identification of Members needs, including physical health, mental health, social services and long term support services and the development of the members Individualized Service Plan or Treatment Plan to address those needs.
The Service Coordinator is responsible for ensuring timely and coordinated access to an array of providers and services and developing solutions to address unique needs of members and coordinating services delivered as necessary and appropriate.
Must reside in assignment area (see Job Title).
PRIMARY RESPONSIBILITIES:
1. Conduct field visits to perform Face to Face Assessments with Health Plan members.
2. Obtain a thorough and accurate member history including clinical, social, and environmental factors.
3. Assess member�s biopsychosocial factors.
4. Identify need for assistive devices/adaptive equipment for members.
5. Assess each member�s clinical, behavioral, social, and environmental information to develop Individual Service Plans and a member support system.
6. Acts as an advocate for an individual�s health care needs.
7. Identify members that would benefit from alternative level of care or other waiver programs.
8. Establishes short and long term goals in collaboration with the member that meet the member's needs and the referral source's requirements.
9. Communicate care objectives to appropriate individuals/departments/ referral sources.
10. Identify members with potential for high risk complications and coordinate appropriate treatment in conjunction with the member and the health care team.
11. Establishes working relationships with referral sources and community resources.
12. Review benefit systems and conduct cost benefit analysis.
13. Evaluate quality of necessary medical services.
14. Acquire data and evaluate necessary health services for cost containment.
15. Document effectiveness of service coordination services.
16. Collaboratively works with other clinical team members and other departments.
17. Participates in Quality Improvement processes.
18. Serve on internal and external committees.
19. Maintain strict member confidentiality and comply with all HIPAA requirements.
20. Other duties as requested or assigned.
EDUCATION AND EXPERIENCE:
Education:
Required:
Bachelors degree from an accredited college or university in nursing, social work, counseling special education or a closely related field or State Waiver.
Preferred:
Masters degree from an accredited college or university in nursing, social work, counseling special education or a closely related field.
Years and Type of Experience:
Required:
Minimum one year of experience working with disabled and elderly individuals in a Service Coordinator, Case Management, or Advocate role.
Preferred:
One to two years working in the community with community agencies. Three years experience in health care, case management, discharge planning, or behavioral health.

Proficient computer skills.
Preferred:
Strong preference for bilingual (read, write, speak) - Spanish and/or Native American Language
Working knowledge of state's Elderly/Disabled Medicaid program.
Certifications or Licensure:
Required:
Preferred: Current State license to practice Nursing, Social Work or Behavioral Health.
Other:
Required: Must possess a valid driver's license, valid auto insurance and access to a motor vehicle.

Must be able to travel locally to member's homes and provider facilities.
